I drove roughly 12100 miles last year. I have had the car only one year and love it!

3. 46.21 mpg over 11650 miles (lifetime)
4. 1995 Honda Civic 4dr sedan
5. 34.36 mpg over 18381 miles (the last 18k miles we put on the car, didn't keep track before that)

Mileage in all cases is calculated by noting odometer readings using the trip odometer at each fillup and dividing by the number of gallons the pump reported pumping in to the tank.

Lets see, in the order asked:

1. My Prius is a 2008.
2. It is Seaside Pearl (the light blue color)
3. My average mileage is 52.3 MPG
4. My Prius replaced a 2003 Ford Ranger Edge pickup.
5. The pickup was getting 22-23 MPG average.

It'll be kind of interesting to see how all this is put together.
