It's on the second page of options. there is a down arrow on the left side of the screen that will bring you to the next page. It took me quite a while to figure that out. Good luck.

What is the second page of options? How do I get there?

I guess I am an idiot or I have the wrong manual.

If you have to read the manual to figure out things, software-wise, it's bad software, things should be intuitive.
OMG, my husband is wearing off on me, I can't believe I just wrote that!

Okay, once your in phonebook and have selected the person/number:

Touch Edit
On the Edit screen, on the top left are two arrows in a circle, touch that and you will be in the voice recognition screen with the REC button on the bottom left. Have fun.

Well excuse me, but none of the people above provided instructions that I could follow; here's how to do it!!!
1-Go to PHONE BOOK screen
2-Press the name of the phone book entry to add a voice tag to.
3-CONTACT DATA screen appears; press EDIT at lower left.
4-EDIT CONTACT DATA screen appears; press circle at upper left.
5-2nd page of EDIT CONTACT DATA screen appears; press VOICE REC at upper right.
6-VOICE TAG screen appears; press REC button at lower left.
7-A voice on the speaker tells you to speak the words you want recorded to become the voice tag for this phone book entry.
8-After you finish speaking, the tag will be played back to you on the speaker.
9-Press OK to verify.
10-Press the return arrows on the previous screens to go back to the
PHONE BOOK screen.
11-Repeat steps 1-10 as needed.

Now wasn't that intuitive and obvious? I guess that's why it isn't in the durn owner's manual....

By the way; the software doesn't recognize my voice tags hardly at all; it selects the wrong thing 70% of the time for me...maybe THAT is why the manual is so cryptic...they don't want you to go there...lol.

PHP Code:
3-CONTACT DATA screen appearspress EDIT at lower left
WOW, I do not see a "contact data" anywhere....
It is also frustrating that I cannot add names to incomming call numbers.

Does the 2005 have a different phone setup from other years?

hi all,
even after skimming this post, this a.m. i tried to attach voice tags without luck - i just printed wa6itd's instructions and hope that helps.
the prius does MANY things well - but, some of the menus/electronics are hardly intuitive
thanks again!
