Well all the lights are on for mine as well as of 2 days ago... checked the 12v, was at 11.8v, tested under accessories load (without engine on) and went to 11.4v then tested with the car on and we're at 14.2v
So just to be safe I went ahead and got a NEW 12v and installed it yesterday. All the lights went away immediately. Drove 30 minutes to the town over after the install... fine.... it sat off for about 1 hour.... drove it back home another 30... perfectly fine.

This morning the wife takes it to work, starts the car, all the lights on again... turns it off completely, then turns it back on and all the lights are off and then drove it to work just fine.

Sits for 8 hours, turns it on to come back home and the lights are on again, turned the car off and on again... but lights stay on this time. Looks at the battery image on the screen and it's at 1 bar. So she lets it sit for about 10 minutes and the battery gets back to the center and top bars... drives it to Autozone, they try to pull Codes but there are NO CODES. What? How can there by no codes?

So turns the car off, goes back into Autozone for a couple minutes, then back out... turns the car on, batter onthe screen is all the way down again and decides to just try and take off, and there is no pickup... dosent go anywhere for about 3-5 seconds then the engine kicks on and then it moves. Then when getting to the stop light around the corner, the battery on the screen still isnt past 2 bars, and does not want to move off the line... then the engine kicks in and goes.. then charged and was fine getting home however the vent in the passenger rear shoulder area was blowing on high, I assume this is the Hybrid battery cooler? It was going non-stop the entire way home on the 20 minute drive she tells me, plus i never heard that go on before, for the 5 years i have the car.

Now the wife tells me after she started the car trying to leave Autozone, the main screen displayed BATTERY NOT CHARGING. Then it went into Neutral and would not go into park. Had to hit the power button to shut it off, then started it back up and it ran fine again.

    Car Specs: 2005, 155k miles.

Is the Hybird battery shot? Do I just need to reset something cause I put a new battery in or what?

Also, I'm trying to find a Bluetooth OBD2 Reader for it, however the only one I can find that says it's Specifically for Toyota Hybrids is this:
Tachyon H1 OBDII Scan Tool for Toyota Hybrids | Tachyon Technologies
But it says its for 2007 and up, while mine is a 2005... I tried contacting them however I am not getting an email back nor is there a phone # to call. I checked out a lot on Amazon as well and was planning on using Torque Pro like many of you use, but I could not find a scanner that read the Hybrid codes, just the standard OBD2. Does anyone know of any other readers out there? I'd rather put the $ it will cost to read the codes towards a scanner instead of driving 2 hours to a dealer then having them charge me $150.

    Thanks all!

Best tool for diagnostics is mini vci cable with techstream on Amazon.

Sounds like a bad hv battery. What state are you in?

yes that will work. Make sure you check the seller, and they are in the US. If you order from China, it'll get to you in a month

Here we go, this ones actually fulfilled by amazon

So as most of these posts say, the majority of these cables do not come with the correct software... where exactly do you get the latest software versions from?

And this will display all the regular OBD2 data and the Hybrid Battery fault codes as well?

    Thanks again!

I'm running an older version of techstream for my Prius cars. The newest version didn't work well........so why upgrade when the older version does everything you need?

I'm running 9.something. The newest is 10.something. There are many sites you can download it from. Someone will probably post it if you ask

It will not matter what version you have.
    Any version will do.
You may not see codes if for some reason there is a communication error with the ecu.

Techstream will show you the most accurate info, including if for some reason you cant connect to the ecu.

Ok, got Techsteam working... and this is what a Health Check pulled.
I take it the only thing wrong and what is causing all these issues is Cell Block 10? How do all the rest of the readings stack up to an operational Prius?

If just 1 cell is the issue I'd like to rip it apart and recondition, any help with a How-to?
    Thanks all for the help!

Well, clearly block 10 is what you'll want to address this time.
Next time you have a failure it will likely be a different block.
As you take things apart, be sure to note which nuts/bolts go where. Some look very similar but aren't. Also some are different lengths.
Get some zip lock baggies and label them.

Because block 10 is basically in the middle of the pack, that kind of sucks. Means you'll really have to tear into things. Charge up the pack to green before you take it out. Then let it cool before taking the end caps off. Otherwise you risk opening it up when all the modules are too hot and they will expand.
