    Hi there,
when I backed up yesterday, some dash symbols showed up. After turning off the car, I am not able to get it in Ready Mode. Only solution so far: to disconnect the 12V battery and wait approx. 2-3 minutes. After that, the lights are off. The 12v battery does also indicates normal voltage. IG-OFF: 12.7V IG-ON 12.2
Any ideas? Unfortunately I cannot get the trouble codes with my scan tool :-(

Seeing lower voltage in READY shouldn't be possible, should jump up to around 14 volts as the 12 gets recharged...unless something is wrong with the DC-DC converter. Are you sure that was in READY mode?
As far as the issue itself, maybe someone else here has had/seen the issue and will be along shortly.

Edit: I was skimming too fast to read that the 12.2 was in IG-ON and you couldn't get to READY. My bad :/

Unfortunately, all those icons don't give any good indication of the source of the problem. Only a Prius capable scanner will give the sort of info that can return good advice. However, if the problem happens again you can try getting "blink codes" from the ABS system. Do a search for blink codes for more info, but the short direction is to short pin 4 and 13 of the OBDII connector, and then make the car ready. Look at the series of blinks in the icons, and list them here.

Did the lights come on when you applied the brakes? Try listening for the brake actuator motor when you step on the brake pedal, and see if it is making any unusual noise.

The lights do not come on every time I apply the brake. Today I disconnected the battery, then we had been driving for about 15 miles, when the lights came up again. Everything works as normal. The battery is being charged, the brake system seems to work properly as well. When I try to force the problem by pushing the brake pedal, it does not work.
I will try to get the blink codes tomorrow or later today.

Ok, got some blink codes, hope this helps.
((!)) and ABS and tire pressure and air bag started to blinking in IG-ON.
ABS and tire pressure and air bag continued blinking while ((!)) started the following cycle: 5 - pause - 8 - pause - 5 - pause - 8 - pause...

This translated to DTC C1259, which means the skid control ECU cannot communicate with the hybrid vehicle ECU. I suggest you seek professional assistance as other DTC probably have been logged which will help to pin down the problem.
