I have another battery I just charged up at home. I'll bring it to work and install it in the car. That way the car will reset itself at the same time. Try everything after the reset. A miracle may happen

But the battery in the car did measure 12.3v so I don't think that would be the problem. But it's good to rule it out.

    Where to I begin....

Changed out the battery, the car started right up. All the warning lights were gone for 1 minute. The hybrid battery meter before the battery change was 3 blue bars, after the battery change, it went up to 6 bars. Kept it running for a couple minutes and the red triangle and another warning light came on. Total 2 lights. Shut down the car and it won't start up anymore. Just like back to step 1 all over again.

Disconnect the battery for 5 min, reconnect and try again. Report the results here. I'm predicting it will start again and repeat the same thing but, I want to make sure before we go further. If it does, you will need to get a Prius Aware Code Reader before we can go further.

Disconnected and connected after 5 minutes. Car started right up again. However it takes 2 pushes of the start button. First push it booted up, 2nd push (both with brake pressed) it started.

This time around it went through the entire warm up process, no warning lights. The ICE engine has turned off and it's still staying on. Been 5 minutes

Drive it around the parking lot and see how it drives. If it drives fine, pull over, turn it off and wait for about a minute or two and try to start it again.

Just went back to the car, the warning lights are back. Shut down and can't restart again.

Ok, so now we know. You will need to get a Prius aware code reader and read the codes. That is the next step. This still may be caused by the brake light switch. Unhooking the battery is clearing the code, allowing you to start it, then the ECU recognizes the problem and throws the codes. Then, the code is stored and won't let you start it.
    Going to lunch!!! :)

Strange enough, when it came back from the dealer the red triangle was not on. They somehow had it turned off. After the reset of the battery, it's back. You think there would be different codes from a few days ago?

What was the other warning light?

What warning messages if any appeared on the MFD screen?

When are you going to test the brake light switch (and replace if needed, with the correct Toyota part)? You need to move past that particular issue to gain more insight into your car's issues.

The other warning light looks like a steering wheel with line running through the middle.

I have a regular obdII that I hooked on, erased all the codes and the car was able to start again. I'm able to start and shut down as many times as I want, as long as the red triangle has not appeared. Once the triangle appears, it won't start again.

During the "on" time, I moved the car forwards and backwards, seems like it moves without any problems.

Please take a sharp photo of that and post.

So you should drive the car around the block and see how far you get and how fast you can go.

The red triangle comes back pretty quick. Might go into limp mode? Here's the 2 warning lights uploaded.

The amber warning light has to do with the brake system and is not related to the steering system. That is not surprising since we know your car has logged a DTC associated with the brake system.

I'm kinda stuck at the office today, no real testing tools available. I can do it over the weekend when I'm at home and have more disposable time.

The brake switch isn't the cause of the red triangle right? That has to be a much more serious issue.

The red triangle is also known as the master warning triangle light. The point of that light is to call your attention to the fact that the car has a problem. Then you are supposed to look around at the other warning lights and messages that may appear on the MFD screen, to try to figure out exactly what the issue is.

So at the moment, since you see only the red triangle and the brake warning light, the diagnostic systems are aware only of an issue with the brake system.

Later on, you'll probably see the hybrid vehicle warning icon appear on the MFD screen, which will show you that you have a problem with the transaxle.

Should I leave the car on for 30 minutes to get all the warning lights to come on?

I don't see the point of that right now. You know that the car has serious problems, and you have no way to independently retrieve the DTC.

I'd rather see you address the known problem of the brake switch. Then see what happens after that issue is solved.

You are correct, I have to correct the problem 1 at a time.

I did however drive it around the block with my OBDII reader in hand to reset if I need to. It drives ok, even with the triangle on, it still drives pretty normal. One thing I did notice, whenever I release the brake, I heard a squeal that comes from behind the car. This happens every time. It's a momentary squeal, a fraction of a second....when the brake is released.
