We were starting to accelerate up a hill, about 30-35 mph when it seemed the car was trying to change gears and it shook a little and stopped responding as normal to the gas... we were able to finish driving up the hill (slowly and only another 200m) to turn around and go home. These are the dash lights that came on.

What could be the issue, and how costly could this repair be?
    160k miles
I still owe $10k on my loan... wondering if I should just find a different vehicle and roll that remaining loan balance into it, or pay for the repair.

    Thanks for your help!

What does it do and sound like if you start and attempt to drive it?

Often even the codes are not enough, especially if retrieved with a cheap code reader because a Prius has lots of special codes.

As far as costs, your worse case would be the engine. Around here you can get a new engine from the dealer for $7200 installed. Head gasket, inverter, or a HV battery for much less. With Ready up its not likely to be a battery or inverter. Possible its a sensor or a slug of oil from the intake that caused it for less.

If it won't drive, in most cases it makes sense to spend a couple hundred for a dealer diagnosis and tow (if necessary) and know for sure. Many car insurance policies will provide a free tow. At that point you have better information. One thing for sure, at 160,000 miles and if can't drive, you won't get much as a "mechanics special". Maybe $1k to $3k depending on whats wrong.

It felt like it was only in EV mode and wasn’t going to kick into gear.

You need to pay Toyota to do a diagnostic before you decide what to do next.

IF.....it still has the original 12 V battery, you might get lucky and only cost you ~ $200.
OTOH, with that mileage if the HV battery is going bad it might be better to trade.

BUT......at some point, you need to have a SERIOUS discussion with a financial adviser about
owing $10K on a car with over 150K miles. NOT GOOD.

It would appear that you are digging yourself into a BIG hole and "rolling" that balance to another vehicle
would just be making that hole even deeper.

D5CF6AE9-EE3E-49DC-8C03-97270E25CFC5.jpeg It probably was in EV mode with the engine off. Often the lights do not point to the root cause. However they do mean something happened. Does it still drive?

I agree a diagnosis is essential. My guess is the HV battery is ok as he states it was probably running on EV mode only. The lights seem to show a dead engine as he states. In my experience if the car is in Ready mode as shown the inverter is supplying 12v operating power and used the 12v battery to get there. Hard to help if he does not give answers to questions like what happens now. Does it go to Ready, does the engine start, does it drive?

It starts and we backed out of the driveway. Couldn’t accelerate past 10mph. Its a slight downhill so it got up to 19mph. It’s in gas mode, won’t switch into EV. Getting a tow to the local Toyota dealer service.

My assumption was that he was running on the ICE only, without battery assist, since the engine shutting down should make an obvious difference in sound levels......and he probably would have noticed that and mentioned it.
    But assumptions are often wrong.

Seems I have the exact same problem. Didn’t have any codes checked yet but was not entirely clear on the final outcome of this situation. What ever happened?

    2102 Prius v, 113K miles,

I am the second owner of a 2013 Prius v. Yesterday when I was driving on the freeway, I felt a surge and the exact warning lights shown in the original post came on. I quickly figured out that the hybrid electrical system was no longer contributing to power the car. It ran okay on the gas engine to get me home.

Today I took the car to the nearest Toyota dealer. Their diagnosis for repair says: "Perform TSB, Repair, Replace IPM and MG ECU Current Sensor."

The codes they detected were P324E and P0A94.

The service agent informed me that Toyota had extended the warranty on the hybrid system and these repairs fall under that warranty. So my car will be repaired at no expense to me.

    My car has ~87,500 miles.

I hope this information is helpful to someone.

    UPDATE:

According to the final invoice, Toyota replaced the following:

    Computer Sub-assembly M (G920H-47040)
    Transistor Kit, Power (04899-47070)
    Plug Kit, Inverter (04899-47060)
    Sensor Sub-assembly, Inv (G920J-52010)

They had my car for 39 days because one part was unavailable due to the COVID-19 shortage of computer chips. Toyota did supply me with a rental car (2021 Corolla); however, they did not offer it, I had to ask for it.

The total repair and car rental were covered under the extended hybrid system warranty which for my 2013 Prius v is 8 years/100,000 miles. No prices were shown on the final invoice.

I have found a little more information on this subject. The four illuminated warning lights on the Prius v indicate the car has entered "Fail-safe Mode". It is documented under Toyota's j0v campaign (that's j-zero-v). I'm not authorized to post a link, but a search on "Toyota j0v campaign" will find the full document. It also explains the loaner car policy under the campaign.

Here is the page showing the warning lights:
