Strange Set if Codes on 2009 Prius

    My son in laws 2009 Prius is having some very strange happenings with odd codes.

    He was getting gas. Started the car but it would not go into drive or reverse at all. Then the triangle of death appears once he tried to put the car into drive. then a car with the exclamation point shows up.

    Once I got there the codes were P1121 and P3193. The tank was full.

    So I removed the negative from the 12V battery, let it sit for awhile. Connected and started the car, it allowed me to put it into drive and drive it home for him.

    Once home I explained I would be back later that day to look further.

    Now the car would not go into drive or reverse again, no triangle appeared at this point. I let the car run for a while and here comes the triangle. Once again I removed the negative to clear everything off and once again I was able to put the car into drive.

    I drove it down the road and back. Parked the car and turned the car off. Turned the car back on and the triangle and the car with exclamation point shows back up again.

    This time no P1121 or P3193. But the scan shows Monitors N/A 4, Monitors OK 5, Monitors INC 2.
    No P codes at all but the triangle is there with no codes showing this time. Codes Found 0.

    Now I clear out by reconnecting the battery again to see if the codes appear again and nothing.

    This time the triangle appears ONLY AFTER putting the car in drive.

    Sorry everyone I am not mechanically inclined but I am trying to help the kids out.

    Any input would be greatly appreciated.

    How many miles on this vehicle?
    Which OBD2 scanner are you using to retrieve the above OBD2 codes (DTCs), please list the make/model or software App.

    Not all scanners are toyota hybrid compatible, and many DIY mechanics prefer to use a "mini-vci" cable (20 usd) and techstream (dealer technician software) installed on a windows device. There are other scanner options as well. Best to locate a compatible scanner and post the retrieved OBD2 codes (DTCs) and any subcodes (INFs or 'detail codes') here for additional guidance.
    This sounds like a HV battery ground leak (P0AA6), after resetting the 12v, you can only start the vehicle once, rinse and repeat.

    Ran the car out of gas and the coolant control valve has failed.

    use the search forums link up top and search both codes for repair info.

    I did that search, but those codes are no longer showing.

    I looked up those codes right away. Tank is full of gas and he said he was never out of gas.

    Do you have the same stored codes (DTCs) still, or have they gone away after multiple power cycles?

    Is the vehicle currently going into ready/starting and stopping again?

    Reset the 12V. Goes into ready, drive it a block and back, then parked. Wont' go into drive or reverse after a quick drive and back into park.

    Not too mechanically inclined but can attempt the coolant fan if it is that. The battery, not so much I am thinking. Asked around locally and no one wants to work on a hybrid. The dealership wants 350.00 just for labor to "check" the battery. LOL With the right scanner a 5 minute job correct? Going back to the original codes showing the coolant and ran out of gas codes maybe he did let the gas go too low but his pump showed 12.00 range of gas when I got there so it could not be too low. Hoping the coolant issue is the problem which I might be able to tackle.

    The P1121 is a code for a problem with the coolant VALVE that controls flow to the heat storage tank (big thermos) in the left front corner of the engine bay. This is a common fault on a Gen2 and is comparatively irrelevant. It will cause the check engine light to come on but won't have any noticeable affect on how the car drives. Ignore it for now.
    Your first concern should be figuring out the problem with the HV battery. Without that the car is dead and useless.

    Florida is a decent sized state, which part are you near?
    You might shop around, as my local dealer doesn't charge for diagnostics. You could purchase a large bag of "mini-vci" cables for that, or four (4) of the vxdiag versions of such, or two (2) Tactrix OpenPort if you wanted to support a local business.
    Most would be under a minute, and techstream (the dealer technician software) could take a 2 mins depending on the scan.

    Just focus on the HV battery issue.

    Here is a step by step guide that can get you halfway to removing the HV battery : 2004 – 2009 Toyota Prius Plug-and-Play Car Harness Installation Instructions | Hybrid Automotive

    Do you think you could install a new OEM battery from the dealership, a new quality aftermarket option, or possibly a low mile used battery from a recent wreck? (search for and watch a couple of different videos to get an idea of whats involved, overall fairly easy stuff)

    If the answer is no, then you'll need to find a local mechanic to do the installation for you. Which metroplex is closest to you?

    The OBD Lx scanner arrived today and I scanned and got this info.

    Not being mechanically inclined I am posting to see if someone sees something I do not.

    The only code it showed was P1121

    Both batteries seems to look good to me but I do not have enough knowledge to decide.

    Did some research on the numbers and they look fine unless I am missing something.

    Does this take it back to the coolant valve causing the triangle of death?

    I appreciate any advice given.

    Car started up, got into ready mode. Drove 3 miles and back and the triangle came back on.

    A useful experiment might be to peruse the repair manual looking for something easy to unplug or tweak that you know would trigger a particular HV ECU trouble code. Do that, wait for the warning triangle to come on, then check whether your OBD Lx scan tool is able to report the code that you know is there.

    If it isn't, that would suggest better results from getting a different scan tool that can.

    Did you use the new OBD2 device/app to scan the vehicle after the triangle reappeared?

    Post the current OBD2 codes by scanning with the new OBD2 device/app now. (do this before anything else)

    Will the vehicle start a 2nd time, or do you need to disconnect/reconnect the 12v to make it start? (possible DTC P0AA6 or P3009)

    Wonder if you're not able to pull all of the DTCs (OBD2 codes) because of the OBD2 scanner App being used, and if trying a free/lite/paid version of Dr Prius (or Hybrid Assistant) might help, see if either App has a freeware version to test first. Remember that whenever you disconnect the 12v, this will erase the stored OBD2 codes (DTCs) until they're set/stored again in the future.

    Yes.

    these codes I posted are after I started the car up, no triangle at all. Drove it and the triangle reappeared.
    I scanned using the new OBD LX scanner.

    So car went into ready after sitting for several days waiting on the scanner to arrive. Drove it a bit and triangle back on, cannot go into drive or reverse.

    Those guys are over 1 hour from me and with it not running they would have to come to me.

    Doubtful if they make house calls, but they may know someone who does. Thinking 129usd for AAA would be a good investment now.
