Good news in terms of actual battery health... Take the covers off the battery ecu and see what's going on inside there? As in confirm all circuits are functioning. In meantime... You can relatively safely drive car around while clearing this particular code as you drive. But best to fix asap. This part that costs less than $100 may be needed:

I've tried clearing the code, it just pops right back up! I'm taking everything apart again today to check.

Can you post some screenshots of what Dr. Prius app is seeing?

I cannot connect to the app with my reader. I was able to pull the code with my father in laws but he did not leave it. And it plugs in, I am waiting to get a new one through the mail. I live in no man's land so its gonna take a week

In meantime confirm all 14 voltage sensing wires are able to send a current, aka continuity test 'em.

Which OBD2 readers do you recommend most?

I tested the wires connected to the back of the pack, by the seats and the 2 big orange wires connected to the safety lock thing. Both gave me sounds. I'm not 100% positive I've done this correctly though. I'm assuming since I got sounds they work? Do I need to go through and check each module with this test? When I took it out I check and they were all reading about the same around an average of 8.2. I'm completely dumb when it comes to all this I apologize. I've looked and looked for a video to help but there seems to be no real good ones!

Ok, then maybe check battery pack voltage... If your hybrid pack was running low before the snows hit and then you didn't drive for a week or two the voltage on the battery sides of the relays might be less than 200 volts... If that's the issue, the only problem is the need to charge and balance battery pack with a 250v 1/3amp charger.

How would I go about charging it? Also, what would be drawing power from my 12v with the safety plug out of the HV and the white plug pulled from the positive on the 12v?

First, get your multi-tester and set it to high voltage and plug in your orange safety plug and get a voltage reading on pack to decide if that's even a concern/cause of the breakdown. Then if it is:

Easiest way to charge would be if you get your engine running to put car in drive with foot on the brake and other foot flooring the accelerator. This is called a forced charge... Another way to do it is buy a hobby charger for NiMH and charge up each of the 28 8volt modules with a product like this: SkyRC Technology Co.,Ltd.

Tuesday i reset the computer by grounding out the 12v for 10 minutes. It worked. Codes went away and stayed away. I was able to start the car and drive it. 12v was holding at 12.42 for hours HV charged when the car turned on.. ended up going green after constantly starting to make sure it actually started.. Checked it the next day, 12v was still 12.42 and the car started with no issues. Thursday, I drove it out of my garage, no issues. Parked it in front of my house, shut it off, started again later, nothing happened. Went a couple miles down the road to pick up my kids, HV battery was in the green by this time. Got kids, started backing up and the dash lit up with everything and car locked up. Pulled over and couldn't get it going again. Tried pulling the positive plug, clearing codes and grounding out the 12v to clear codes. Nothing worked. Had to tow it home but the 12v kept dying so we had to jump it with the jump start up front. I am so completely lost and desperate for help. I still don't have a compatible OBD2 yet and my farther in law can't make it out till Sunday.

Grounding out the 12 volts? What does that mean?

you sound pretty lost. There’s thousands of posts about hybrid battery repair
On this site use the search forums link up top and search

    hybrid battery.

1—- All 12 volt battery measurements should be done from the jump point under the hood that’s the only place that voltage level means anything.
pretty obvious 12 volt is bad but measure that jump point with car off.
Then with car still OFF turn on the cars headlights in high beam and while there on measure the front jump point again. What are those 2 voltages?

Then pull the orange safety plug on the hybrid battery and r3move all the connectors on the ecu. Then remove that ecu can. It’s the slim box can about 9 inches long. It’s shown in the second YouTube you posted.it has the sticker on it.
Look closely in its plug sockets for corroded pins. Then take its cover off look closely on the backside of the plug sockets for corrosion. Hundreds of posts about this onthe SEARCH FORUM LINK.

If no rot you prob have bad battery modules which is just about the most common thing ever on this site. Start reading posts. Easy to test.
