Yeah I did the job in the summer of '17, took me a few days because of work, sleep ect. I'm just not looking forward to doing it again.

@Dan05979, where you able to resolved the issue?
I'm having same issue but worst than you.
6 month ago, I did clean intake manifold and ERG pipe, and now I'm having the same type of your issue.

    Anyone have any idea. ?

Mendel, I have not clean the EGR cooler, because it seem very hard to remove the cooler.
I did clean the manifold and EGR Valve and change spark/coil. The car did run for 6 months and now the issue is shaking.

Cleaning EGR is my next steps.

Maybe start by taking the lower retainer nut off the cooler. It’s hard to get to, but it doesn’t require any collateral disassemble, and everything is ok with it gone, nothing’s loose.

it’s pretty much the most difficult thing to do, and you can just leave it off.

Info in first link in my signature.

Cleaned EGR Cooler, Throttle and manifold with adding Oil catch can.
The car start without any shaking and run well. But there is a white smoke from the tail pipe. (small smoke).
The car have 255K miles and I be leave I have a blown head gasket.
I will try to add head gasket sealant since I think its small leak.

Anyone recommend the head gasket sealant ?

Not me. It only delays the inevitable, and gums everything up too.

Thank you everyone for your guidance.
I decided to replace the head gasket, all head is out off engine now.
    Here is my situation now:
    1. What other gasket or O'ring do I need to replace ?
    2. which area do I need to put the RTV silicon and what type of RTV ?
3 Torque for Head bolts ?

    Please advise.
    Thank you.

For #1, Toyota sells a gasket kit that includes head gasket AND pretty much every pliable gasket, o-ring, and seal. The attachment lists the part no for that kit, as well as for head bolts. Replacement of those bolts is not mandatory, if they're still within specs (see following paragraph, link to Repair Manual excerpts), BUT many do replace them anyways, just to play it safe.

For #2/3, in my signature there's a couple of head gasket repair links. One has a number of relevant Repair Manual excerpts, should have all the torque values, sealant spec's and application instructions. In this video Gasket Masters use an alternative sealant. Lots of tips too, the head gasket bolt torque for example:

Another by Gasket Masters, what to do if head bolts are not coming loose:

The other link is to a thread by @The Critic , and he may chime in as well (having been pinged) regarding appropriate sealant for starters.

Full disclosure: I've NEVER done anything remotely as difficult as this myself, just an armchair mechanic.

Send the head to a machine shop and have them verify it. In most cases warpage causes a coolant bypass under or over the metal hg. In your case, white smoke indicates its been bad for a long time. Others have had a hg job fail quickly by skipping steps.

Thank you, very good idea. I will take the head to the machine shop. My plan is not to skip any HG process.

Here's a couple more head gasket videos that may help:
