Drivers side wheel cylinder hyper-extended again, happened before about 8 months ago. In both cases it seems that braking while in reverse may be the cause. Not sure why this would happen, all hardware is correct. Ideas?

What do you mean - "hyperextended"? Pictures of the results?

I don't think that happens when all hardware is correct, so it would spur me to carefully recheck assumptions.

One thing that can easily happen is adjusters get swapped left for right (or just one gets replaced, and isn't for the correct side). They look almost identical, but the thread directions are different. Any one that's not on the correct side of the car will auto-loosen instead of auto-adjusting.

The piston (one side) was pushed right out of the cylinder.
