Installed a reconditioned hv battery about a month ago. Things seemed good. Now I’m getting this, lasts about 1/2 second. No codes.
    Ideas?

There's corrosion on the voltage sensor pins inside the hybrid battery ECU and it's thick enough to start shorting itself out with micro arcs of electricity. You might be able to clean this, or if it's already fried a used one online is about $100. Also replacing voltage sensor harness with brand new would be wise to do. It's all because Toyota's Nickel based battery technology is prehistoric and it leaks toxic electrolyte at the seals where the terminal connect. Once you get this all cleaned up it won't be an issue, but hopefully the person who sold you the reconditioned pack will deal with it. Eventually you'll get a P3000 error code.

So it turns out my OBD reader was failing, these readings are false. I did a full look-see using Techstream and all is well with the battery. I have a replacement OBD reader on the way.
