EGR valve replacement now car is death shaking on startup

    2013 Prius V
    245K
    Head gasket HAD BEEN REPLACED at @ 230K
    Car was running fine when receiving EGR CEL, no issues.
    Car still ran fine after receiving EGR CEL.
    Immediately ordered New EGR Valve.
    Replaced-
    EGR Valve and PCV Valve
    Cleaned-
    EGR pipe
    EGR cooler (NASTY)
    Intake Manifold

    Started car back up and got the death shake, it would not go away.
    When I put the car in neutral when running it definitely reduces the shake, but still not running well
    P0300 P0301 P0302 codes (received at different intervals of diagnosing).
    Checked Spark plugs and coils, not fouled/working improperly.

    I think I have a Vacuum leak issue (forgot to put a hose on or incorrectly)
    I want to get some feedback before I tear it down for the third time.

    were they oem parts from a dealer?

    Is this right:

    head gasket replaced at 230k miles, but EGR not touched, and likely never cleaned.

    EGR cleaned at 145k miles.

    If so, my guess: head gasket blew again, due to the 15k lag of EGR cleaning.

    This is your evaluation of virtually every Prius on here with high miles = Head gasket failure.
    The car was literally running fine, 24hours to replace and clean parts, then put back on and death shake on startup.
    No way a head gasket failed within that time span.

    Yes.
    Clean/replace EGR components won't stop a current head gasket leak. You replaced head gasket, left the EGR untouched for an additional 15K.

    How did the EGR look btw, say the cooler, at 245K.
