I spent weeks determining the best replacement HV battery option for a ‘12 with 130,000 miles. Settled on purchasing a local rebuild from a different model ($600) I was to use 20 modules in mine. However, I was outvoted by the lady upstairs. So we purchased a “geak” version with 3 year warranty for a total of $1800 (new 12v included).
He came and went…..I test drove, only to have the “system warning” come back on. Barely made it home from a 19 mile round trip.
I screwed around with my cheap code reader….clearing and re-clearing and….
For some reason I opened the hood and noticed the hybrid coolant reservoir was dry….topped it off….cleared the codes again. The next test drive was FLAWLESS!!!…….Until I got back and noticed the coolant puddle.

I’ll see if I can find the leak (hopefully an easy to get to hose) later.

Oh, the tech did ask me if I had it diagnosed prior to disassembly (remember I was going to do it myself).

Conclusion — Read The Title of this post.

Update…..Can’t find the leak. Doesn’t appear to be an easy to get to hose. Will be making an appointment…..probably at the local Toyota dealer.

Have you checked the reservoir tank? I've seen people replace the OEM spring hose clamps with something else and crush the plastic barbs or connection tees - creating a leak after a few warm-up cycles or hot/cold seasons.

I haven’t, but I well. That said….I’m pretty sure it’s coming from the radiator area. I took the plastic ‘under guards’ off, just couldn’t find the start point of the leak. I may have to drive it (get it warm) and observe then. If it’s not a simple hose or tube, it’ll have to go to a garage.

OK….car went to the local dealership. Somehow, in the ‘text authorization’ (this is my lady friend’s car _ she made the appointment and “authorizations”) we ended up with the repair ($815 + $174 for the flush). We also got an air filter ($41) - I have a stack of these in my garage. We got a cabin air filter ($49) - I laugh at folks who pay for this service. We received an engine coolant system flush ($132). New spark plugs ($187) - I also have a set in the garage.
All told $1560 dollars — nearly double the actual REQUESTED repair. All she saw was the quoted total = not the itemized breakout.
Very disappointed in our small town dealership group.
I did discuss “document fees” with them when I purchased the most recent of three cars. I mentioned they were becoming ‘citified’
They are now full on CITIFIED!
This will be filed under the; Screw Me Once….Shame On You / Screw Me Twice….there ain’t gonna be a twice.
Oh….and I drove MY ‘12 for 460,000+ miles without any of the above services except the air filter replacements (done in the driveway).
I did write a GOOGLE review …. As they requested.

Sorry to hear what happened to you (or your lady friend). What was the cause of the leak finally? Did you ever change the coolant before the leak following the Toyota maintenance schedule?

They replaced the radiator assy, hose assy, hose, hv water, clip, hose, hose, radiator, clip, hose, radiator - all these with part numbers.
I really don’t know which was leaking….but I suspect it was the radiator. No problem with this part of the service. And, no….it had the coolant it had when the car was purchased used in ‘15 or ‘16. I drove MY ‘12 460,000 miles on its original coolant.

To put this to rest, I talked to the general manager (he reached out to me because of my GOOGLE REVIEW__actually the reviews are set up with an auto reply when a negative is posted. He sounded surprised that I actually called.
Anyway, we agreed to chalk this one up to an unfortunate misunderstanding that favored the dealership in its entirety.
