speed dial question

I thought this would be easy but could not find an answer using search on in the manual. 2007 Prius I have placed a few numbers in Speed Dial and it works well. My only problem is one entry. It appears twice in the 1st SpeedDial page - once the icon looks like a house and once it looks like 2 index cards. I want to remove/replace the entry that has the index cards icon but it appears eternal - I can't seem to remove/replace it. What do teh index cards represent? How do I get rid of the entry?
