Car goes to ready for a second then not ready. First we tested the 12 volt battery, it was ok but we put in a newer one anyway that tests better. Still no start. Next we replaced the safety plug with a known good one, still no start.

I did notice no turbulence in the coolant container so we'll try replacing the electric water pump.

Or should I start swapping out resistors and relays in HV battery case? (have several used laying around)

If there's no turbulence in the electric pump, then that's definitely where you should start first. I don't think this is a battery issue.

If I remember correctly, I think there's also a fuse that causes this issue....I'll see if I can dig up more

Yes, I forgot to mention the fuse. I did locate that and it's ok.

so I would say do the pump first. I think we should have a recommendation to change the pump out every 100k as preventive maintenance. There seems to be failures around the mark.

DTC P3004-131 means the safety interlock is not correctly installed or the fuse within is blown. If you are sure that the fuse is good and that you are correctly installing the interlock, pivoting the lever 90 degrees then sliding it down to lock in place, the next place to look would be the system main relays and the current limiting resistor.

Although the inverter coolant pump may not be functioning, that would not cause P3004 and has no effect on the powertrain when the inverter is cold, as would be the case if the car has not been driven for a while. So I would not worry about that until you've figured out the root cause for P3004.

Thanks for the response Patrick. Yes the safety interlock is installed correctly and I've tried another off a working Prius with the same result, no start. Is the system main relay and resistor the ones inside the HV battery case or elsewhere?

There are three system main relays and one current limiting resistor, and they are located within the traction battery case.

Did this problem ever get resolved?
I have replaced the HV battery pack in our 2007 Prius and have the same trouble code
    P3004-131

I've got the same thing going on for a 2005 Prius (P3004-131). Still searching the forums for anyone who's had this problem and resolved it, but no luck yet.

Let's start with how did you get to this point.
You were driving along and then it came on for the first time today?

Good point - thank you for responding and asking! Here's the whole story.

2005 Prius, 197k miles. Around 155k miles red triangle and check engine light came on and my cheap pocket OBDII reader said the error was P0A80. I replaced the hybrid battery with a reman from Cardone and had no problems for two years - then P0A80 came up again. The battery was under warranty still, so they sent me a new remanufactured battery. I got two more years out of it, and then got another P0A80. This time it was no longer under 3 year warranty, and I was hoping for a battery swap that would last longer than two years.

I ordered a rebuild kit from Electron Automotive and swapped all the cells. When I put the battery back in and started the car this time the red triangle, check engine, traction control, and (!) lights were all on. OBDII did not show any errors, and I was stumped. I carefully checked all the connections and the orange plug, but the car still showed the same trouble lights on the dash. I pulled the cover and checked torque on every cell connection as well. Oh also the ICE would not come on and the car would only go into neutral.

Next step, I got a USB cable and TechStream. Live data for hybrid battery works and shows voltage for all the cells. Running health check in techstream showed the car had the error P3004 with the sub-code 131. Googling for the code got me to full diagnostic manual for the Prius which shows "HIGH VOLTAGE POWER RESOURCE MALFUNCTION" So far I've verified the safety plug is working as expected (fuse not blown), and the system main resistor measures right in the middle of the expected range (20Ω).

Hopefully today or tomorrow I'll have time to pull the relays out and test them, but I'm wondering if there's a specific fuse I can check, or anything else that might commonly cause this?

Well, disregard my question - I figured it out! While putting the battery back together, I failed to connect one of the main power cables. In pulling things out and turning the case over to remove the cells more easily, the short main power cable had gotten tucked under the other cable and I didn't notice the terminal tab when I was (hastily) putting everything back together.

That is a mistake I will not make again!

Thank you so much for the follow-up!
Too often we don't hear back and are left hanging.

Even making a mistake is a great opportunity for us all to learn something.

Yes. For what it's worth, Electron Automotive was SUPER helpful over the phone. The tech gave me several things to check on, and finished with "if that doesn't work, take it apart and put it back together again". That last bit of advice was best, and that's about where I was when I discovered my mistake.

Which power cable did you mean? Is it the main +ve and - ve power cable that feeds into the relays , the relays which is connected from the cable coming from the front of the car?
The power cable connection are fine in mine. But still no start.

I'm on the same fault p3004-131.
I own a second gen 2005 Prius with 200K miles , owned it for 5 years when it was at 160K.
It was all good until a year ago when it was at 195k miles the Hybrid battery started acting up. I have since swapped hybrid battery modules twice - once for block 5 and once block 14 in the past one year. Also changed the transmission fluid - genuine Toyota.

As again- this time the battery gave a block 11 low voltage error but the car was driving with the error code of course. And when I changed the Block 11 this time everything went south. Before I changed the module it never showed the p3004-131. I was confident that after I changed the module it would run like it did before when I changed the other modules. Is it that I messep up the connection while fitting them back I don't know , but I have done it couple of times before so I think I have done them all correct.

I have nothing else except multimeter for diagnostic on top of the Techstream miniVici. Is there a way to test these relays. As far as I can visually check they look clean. It's not like they are fried up like in some videos I see.

The limit resistor stands correct at 20Ω.
The continuity on the main orange safety interlock is good.
Each of the Hybrid modules hold good voltage all above 7.7V. And even the Techstream shows above 15.5V for each block.

OK. Here is my update again and I found the problem. Basically when I put the battery back a m4 hex nut was not screwed all the way down on the first hybrid cell. I had put them hurriedly using a handheld screw gun. The stud was damaged and now I cannot screw it. Anyway I just torque tied a nut adjacent to it and my car started. I'm getting a chamfer remover tool to fix the stud.

The car is running fine but again as I ran the Techstream I have found a new error P0A0D -350. And the P3000-123 error. Though the car is running good and no any Triangle sign - the Techstream is scanning this error.
