    Aftermarket accessory. Now way would Toyota allow such a device to be poorly installed and placed.

    Definitely aftermarket. And what about that doohickey on the left side of the steering column in pic #2?

    aftermarket bluetooth microphone? and maybe the thing on the left is an activation button for said bluetooth?

    The think on the left appears to be the GPS antenna; looks just like the eBay image in this link:

    The thing on top is likely a microphone.

    If you search around, it seems some people have figured out a hack to get a non-Japan Map working on the Japanese GPS head unit. That being said, I would be cautious, as people on the Internet can say and promise anything.
