Car won't start- Codes POAOF & P3191

    Looking for some answers to the problem. I have a 2004 Prius with 146K, it was abused in the past, but was running OK since I got it last Oct. 2017, but with some quirks and warning lights coming and going. So, I have recently been working on it- tuning it up for fear of being stranded on side of the road. It finally happened a day after the tune-up on the way to airport to help a friend. Since then, I changed out the 12V battery with new one and also reconditioned the HV battery which was a long process but I am pretty sure went well using Hybrid Automotive's charger and de-charger. I also cleaned up some corrosion on the battery connectors as well. I have read many posts on here and done all the obvious stuff.
    Unfortunately, still No Start and throwing these codes:
    Hybrid Control- POAOF- 205 Engine failed to start
    Engine & ECT- P3191- Engine does Not start
    This second code gave me several pages of freeze frame data to review, but nothing jumps out at me or I don't know what to look for to direct me to the problem. What should I look for here?
    My next plan was to check the fuel pump to see if it is working. I checked it last week and I believe it was working but now sitting for a week, time to check again.
    Any other suggestions? I am still but an apprentice in your realm, but willing to learn.

    Can you post screen shots of your freeze frame data?

    What is the 12 volt reading measuring after sitting overnight?

    How many miles have you put on since the reconditioning cycles?

    The current charge readings on the new 12v is 12.16, could that be too low?
    And to charge it- is a standard charger Ok to use with these batteries?
    I think I read a charge rate of 4 amps?
    Screen shots are new to me and the data is on a separate laptop for car-
    so that may take some work.

    No miles on it since reconditioning, still not starting since breakdown with all warning lights ablaze
    I did read one post saying the HV battery may have too high a charge after reconditioning for car to accept as normal and start vehicle. Hybrid Auto says no likely an issue as battery charge lowers down after charge and after start attempts.

    And I just confirmed that the fuel pump is working.

    12.16 is low:(.

    You can use watcha got, just keep an eye on it;).

    What I would do is take pictures of the screen and readings off of Techstream with your phone them post them here :).

    Yeah I’ve run our 2010 after it has sat for a couple of hours and it’s been fine ;).

    Just to walk through what you did:

    • How many reconditioning cycles did you do?
    • What voltages did you go down to?
    • Who installed your harness?
    • Is the orange plug seated properly?
    • What is the voltage on the pack reading?

    I did the reconditioning myself following Hybrid Autos instructions
    • How many reconditioning cycles did you do? 3 cycles of up, then down
    • What voltages did you go down to? 140, 80, 20
    • Who installed your harness? Me-
    • Is the orange plug seated properly? yes
    • What is the voltage on the pack reading? I completed at 239V, but have not re-checked
    • I am uploading shots of tech stream data

    This should not be a contributing factor to your no start situation. However, purely from a lead/acid health and maitenance point of view, this is nearly fully discharged and needs to be charged to full ASAP.
    It is better than nothing if that is all you have at hand. But, AGM batteries (if you have the OEM installed) require a charger with an AGM setting as it requires a slightly higher charging voltage so that it can lift the resting voltage to 13.2 V. If you don't own a modern multi stage automatic charger with an AGM setting you could start budgeting for one now. They are $30-$50 and upwards for something decent.
    Yes that is Toyota's recommended maximum. I guess they don't want it gassing off with a too aggressive charger.

    That is all about the 12 V battery, but does help with the no start. That just comes down to the basics, just like any other ICE – fuel, air, spark.

    Yes, should have asked Napa if it was fully charged- assumed. I charged it up to 13.5
    Yes, I know the 3 positions of the orange lever.
    No, does not get to ready - it's a weird sequence-
    I have to press power twice, nothing on first push
    then I hear the electrics turn on, after a few seconds-
    then I hear what sounds like the motor kicking on then sputters out hard after 5-10 seconds,
    then it kicks on again for a few more seconds and dies a second time as the warning lights come on.
    Any thoughts on my next steps?

    The detection conditions for these two codes are:
    2006 Prius RM - DTC P0A0F-205 Detect Cond.png
    2006 Prius RM - DTC P3191 Detect Cond.png

    I have attached the Inspection Procedure charts for both. Interestingly the P3191 directs you to fix any other code, before proceeding with the inspection for P3191. The procedure for P0A0F-205 is short and sweet.

    Problem solved- Blew some air down the throttle body and out flew a small leaf or particle from cleaning rag used to clean the throttle body, caused a fuel blockage, once cleared all issues resolved.

    I Spoke too soon, some follow-up codes have come up that I need to address.
    Please forward the trouble shooting steps to eliminate these issues:
    HV Battery- POA85- Cooling Fan 1 - Control Circuit High

    ABS/VSC/ TRAC- C1310 - HV System Malfunction
    I'm guessing these may be the same issue.
    I plan to check the fan motor but what's the best method?

    Check the harness to ensure it is clipped and locked;).

    Also check to see if the fan has any build up in it as that could cause it to malfunction too.

    You could also check to see if it is running by putting a tissue over the vent and see d it draws it in ;).

    Those are the simple things:).

    Thanks- I found the fan connection was not securely snapped in. Had already cleaned the fan.
