P2195 Code, bank 1 sensor 1 stuck lean

    I am working on a 2012 Prius V Five with 177K miles. I just bought the car recently as a fixer upper for the family. This is my second Prius, my other is a 2010 Prius Three. Back to the 2012 V, Originally the car would miss under the slightest acceleration. It took me a while to diagnose that issue because it produced no error codes. New plugs, swapped ignition coils, injectors, and egr assembly from a parts engine I have. Then I discovered a post about the intake. I found the intake manifold egr ports were clogged with carbon. All cleaned up and running smooth again. It was a relief to find the issue and fix it. Now I can finally drive it. First trip after the egr port cleaning the CEL is on P2195. I removed and cleaned o2 sensor, the light came back on. I then swapped both o2 sensors out of my 2010, my 2010 had zero issues. Again P2195. Ironically my 2010 now has the 2012 V o2 senors installed and it threw a P2196 stuck rich, just the opposite. Swapped back ALL the parts I changed the first time around, still the same result. I bought 2 new o2 sensors from Autozone, installed them P2195 is back. So what is next? Smoke tested, no intake leaks or exhaust manifold leaks. Installed a blind gasket for the egr to make sure it is not leaking, just threw another code along with the P2195. Any assistance or direction would be greatly appreciated.

    Wiring is ok?

    Did you do anything to the throttle body or MAP sensor?

    I didn't have much time to do anything today. I did clear the codes and took it for a short run now P0032 is coming up. That is a heater circuit for sensor 1. I will check fuses and wiring in the next day or so.

    Cleaned throttle body, cleaned MAF, swapped out MAF. I did not do anything to the MAP

    I have the p2195 code too but before it was p2238. I ordered an o2 sensor that will get here tomorrow. Have you found a solution? I just had my engine replaced.

    I have same error with same car but i have about 400.000miles
    p2195.. p2236..

    My car with LPG gas.. (popular in Europe) maybe this is "problem"? but i dont have problem with fuel consumption or engine work.. 100km /4.5-5l

    tomorrow i will check my MAF sensor..

    My 2012 Prius V wagon shot a P0172 code = rich fuel bank 1. I took out and cleaned the MAF with CRC MAF solvent. Let dry a few hours and screwed back in. Also bought new Fram Ultra engine air filter at Walmart and installed. Next is a whole plethora of things it could be. This is said to be the most difficult code, since it could be so many issues. I'm thinking next to try to do the coolant temperature censor, the upbank, or upstream, O2 sensor, which I can likely access by taking off the windshield wiper motor, and the dummy/blocking plate, with those fifty nuts, below that. Once you remove that 'iron bra,' it's pretty much Shangri La in terms of access to so many things. It's the one you gotta remove to do coils/plugs. I did find two nifty plastic clips, which can be done by hand, however, to add DOT 3 brake fluid.

    Anyone ever shot a P0172 rich fuel bank 1 code? What was the repair that got your check engine light to finally go away after shooting that code?

    Thanks everyone for the information. I have never posted before but after going through everything and wasting a lot of time I figured I had to and hope to save someone else a massive headache. I changed the MAF twice, both O2 sensors, cleaned throttle body checked vacuum hose pulled injectors and kept getting the same code. Finally for whatever reason I pulled the spark plugs and they were completely black as can be

    Got posted before I finished. Spark plugs completely fouled black as can be. Change them out with new plugs and issue solved.
