    Current evidence does not, therefore, appear to show a need for boosting in the general population, in which efficacy against severe disease remains high. Even if humoral immunity appears to wane, reductions in neutralising antibody titre do not necessarily predict reductions in vaccine efficacy over time, and reductions in vaccine efficacy against mild disease do not necessarily predict reductions in the (typically higher) efficacy against severe disease. This effect could be because protection against severe disease is mediated not only by antibody responses, which might be relatively short lived for some vaccines, but also by memory responses and cell-mediated immunity, which are generally longer lived. The ability of vaccines that present the antigens of earlier phases of the pandemic (rather than variant-specific antigens) to elicit humoral immune responses against currently circulating variants indicates that these variants have not yet evolved to the point at which they are likely to escape the memory immune responses induced by those vaccines. Even without any changes in vaccine efficacy, increasing success in delivering vaccines to large populations will inevitably lead to increasing numbers of breakthrough cases, especially if vaccination leads to behavioural changes in vaccinees.

    I always get a kick out of "The Lancet," first published in the 1820s...and named for the particular scalpel commonly used at the time for bloodletting.

    1. Many if not most nurses work in places where there are less than 100 employees, and there's something of a nurse shortage. and.....

    B. Doctors run the hospitals, but Nurses make the hospitals run. Only the stupid ones are standing up for freedom - the rest of the vax deniers are just gundecking each other's little white cards. ;)

    I personally think it's a shame because instead of squirting the vax into trashcans 1cc at a time, or letting thawed lots go into the trach because they ran out of arms before they ran out of vax is a waste.
    They should let those who want a third shot.....and there ARE some.....come at the end of the day and get jabbed on a standby basis.

    Some hospitals, pharmas, and other outlets are doing so as we speak.

    The problem "massive or otherwise" is that there are vax deniers on both sides of the political spectrum.

    ...."MY body, My choice!" seems to ring a bell......;)

    This refers only to booster vaccinations (such as a bonus third Pfizer/Moderna or second J/J vaccination). The Lancet strongly encourages everyone to get vaccinated.

    And discourages the booster. The booster makes sense in making the readily available antibody generators 'tweaked up.' I suspect the next variant will make us long for delta.

    I'm happy to get boosted at some point if the efficacy data support it.

    I'm also happy to wait a while on that as long as large numbers of people need jab 1 or jab 2. I may have been put here for a reason, but the reason wasn't necessarily to outlast person X or Y or Z.

    Near as I can tell, vaccine is being thrown out because of the refusal of some to get the jab. As far as I'm concerned, they've stepped aside so I and those like minded can go to the head of the line.

    Yes, I would like excess vaccine to go to willing, unvaccinated people AFTER our team maximizes their resistance. But I live in Dixie where the ICUs are filled up:

    My local hospital is keeping some ICU beds open by instituting a one-stop COVID center in 3 large tents outside the actual building. All comers are given rapid in the car tests and upon a negative test result are offered a shot, on the spot.

    Positive tests are followed up by an offer for an immediate Monoclonal Antibody Infusion.
    Wasted doses are also being minimized since Pfizer "proved" that their doses are stable at standard refrigeration temps for 31 days.
    Anecdotally, demand for testing is fairly high, and the infusions are nearly universally accepted BUT jabs have been trending sharply downward according people I know under the tents.

    My last sinus infection was proven to be non-Covid by an at-home test ONLY because storm restoration has resulted in some of our techs being loaned out to towns and cities that are still getting power restored.....

    Schools (Unlike up north) have been back in session for nearly a month, and all of the oldies have already been jabbed, and parents are just too busy with homework, dance classes, football practice, etc....to be bothered with the whole COVID thing.
    My adopted home town is 38% non-white and many of them are deeply skeptical of the vaccine, including one neighbor, and some that may or may not work for my beloved company.
    Big Bell offers employees to test out in lieu of the vaccine, and at least one family member and more than one employee were forced to take unpaid leave because they got the 'vid while un-vaxed.
    I've told more than one of them that they're sorta moronic but...people under 50 simply have a radically different view on the subject.
    I would say that vax participation in the 50 and under crowd in my coworkers is probably one in 5 or 6.
    My union (CWA) is quietly trying to play on both sides of the street but this is "free soil" and it's cost them a few members in my local.
    I raised the question of children over 12 being forced to get the shot and I received a very robust reaction from one teacher and 2 RN friends of mine at church....using language and tones I do not normally hear on church grounds.

    There's a theory named for a scientist whose name I've forgotten that states that these bugs get weaker as they evolve....which makes sense for an intelligently designed universe.

    We're only in the first few chapters of this novel.
    If history guides.....it's gonna be a looooong novel.

    I'm pretty sure we do see that effect, in the aggregate, in retrospect. For example, with the 1918 influenza virus, which we had some discussion about a number of posts ago and I didn't get around to following up on. Looking back from our comfortable seats in the future, we see predominantly much less virulent flu bugs than 1918.

    But a key takeaway while we're in it might be that the mechanism explaining that doesn't involve any kind of benign-universe guarantee that new mutations as they arise are uniformly less virulent.

    In the aggregate and over time, we see the less virulent ones predominate because the more virulent ones wasted more of their damn hosts.

    My understanding is there are at least two cells involved in responding to a virus. One is immediate and makes great quantities. The other is a ‘memory’ cell that can trigger the antibody factory cells. My concern is the next variant.

    My understanding is the vaccination rates range from 30% to 80%. This means we have a fertile culture for ‘natural selection’ to incubate the next variants. For me, less a question of IF but when. I already live in Dixie with rates around 30%. As for those in New England, you may have a week or two more time to respond.

    No, I’m not medically trained but my engineering background and math makes me cautious.

    I live in a purple state but a very red county. Very good ol' boy. Yet I have a booster appointment on the 28th via the county health department. I'm in good health for a 78 year old so no special claims to the booster. Initial shots were in January.
