Red Triangle Of Death: Always On.
    Check Engine Light: Always On.
    Yellow Indicator Light Always On.

The petrol engine is always running, loud revs sounds especially when stopped, reduced power, also it says on the screen Instant Fuel is 99.9 liters, also in about 2 weeks prior the hybrid battery was discharging very fast, and sometimes would not charge correctly unless the car was turned off and restarted. the fan behind the backseats is now always running at a high speed even when the car is first started.

OBD11 Port: Does not work, checked fuse its fine.

What do you mean the OBD2 port doesn't work it's not really live I mean because you plug something in and you get nothing in return well that may be because what you're plugging in can't speak and have a conversation with the system that's trying to talk to more than likely but that's just a wild guess. If you have the red triangle of death in the fan running all the time something with the HV battery system is happening this is exactly what happened to my car however many months ago bought a $1,400 new battery put my front end in computer on the new cell pack. And there's been in the green and blue ever since I haven't heard the fan come on since which is kind of interesting but you can't hear it usually when the car is running normally and when the fan is doing it's normal job. Until you go and text stream or some software and turn it up you won't hear it I've never felt the need to turn it up in the 100° heat in Southeast United States but then again I'm not trying to run hundreds of miles an hours up and down hills.

sounds like the hybrid battery i'm afraid. you could try cleaning the fan and checking inside the battery for corrosion

If the car is not moving and the engine is running, then by definition it is using an unknown number of L/100 km. It seems the Toyota engineers decided to show this as 99.9 L/100 km being the maximum it can display (even though inaccurate). In the 2006 refresh, the display was updated to display --.- L/100 km in this circumstance then when the engine stops running the display shows 00.0 L/100 km.
