Hello- we have a 2012 Prius V with approx. 158,000 that has been very reliable until last week.
After a series of 20 minute trips all day the car was parked for 1.5 hours. I left for another errand and drove for about 4 minutes. After stopping at a stop sign, I pulled out and gave more throttle to get up to the 40 mph speed limit. The car accelerated but didn't shift into 2nd gear, I slowly lost power and pulled off to the side of the road. I then heard a clunk.

I realized we were in trouble and I hoped I could limp back home. I turned car around and was able to drive another minute or two, I believe in EV mode but with minimum power. The check engine light came on and then the car lost all ability to move forward.

We rented a code reader and the code displayed is P3190. We haven't been able to decipher that code and we're hoping to have more information before we bring to a mechanic or the Toyota dealer.

Anyone else have this experience? Or can you give more info on that code. Any thoughts appreciated.

Basically it means a typical engine problem. One of the most obvious is running out of gas or a bad fuel pump (rare). Have it diagnosed soon.

Yep; there's a problem with your gasoline engine - won't run.
Usually when your engine is warm and you have a decent charge on the traction battery, the engine will turn-OFF when you stop at a stop sign. When you take-off again, the engine fires-up and you move. That error code is saying the engine isn't starting or running on it's own. Continuous attempts to spin the motor will draw down the traction battery, requiring a dealership recharge of that traction battery. Judging from the age of the battery; a deep draw-down could also damage it - push it over the edge.

Thinking outside of the box. Could it be water in the gas and cold temperatures? Maybe relocate the car to a warm place? If possible.

Ice in the fuel tank at these temperatures.
