just purchased a 2009 Pruis for our daughter- went to dealership to test drive and it wouldn't go out of accessory mode-
they thought it was related to the aftermarket remote start as this was the first it had presented with, they had trouble with not being able to unlock the doors to the remote start shut off when they had drove it to the office for us to look at
they looked it over again, couldn't find a reason and unhooked the 12V battery to try resetting it
both batteries had been replaced at trade in earlier in the week
drove it for a test drive and it was great
they delivered it to us on Friday and after they left it did it again, disconnected the battery and it was fine they said she had to hit the remote start fob
remote fob is no longer in her possession
today she was driving it and had a VSC and brake light (circle with ! in it) came on, she pulled over and shut her car off to check what was wrong- went to start it and it was again in accessory mode again
disconnected the battery again and it was fine
help- she frustrated and afraid to drive it for fear of being stranded!

Well where is the remote start remote. Apparently needs to be in her possession. Now you have a $3,000 brake job coming up this reason for scanning before purchase . This codes probably been up before they found a way to clear it and sell it boy oh boy. Hopefully you have some kind of warranty something on a 20 year old car or whatever it is now holy moly. This is a great first car when it's been sorted out. Preferably not on your dime

It’s not in her car because they thought it was the problem originally.

I have the same year car. 2009. Bought it 2024 July with 147k on it. Didn't had the VSA light, but the Actuator was making sound every 10 seconds. Took me a day in total to replace it with OEM part (was ~$1.2K back then). Then all fluids, throttle body, plugs etc. Now car runs good.

Battery and the ABS actuator are the big ticket items on these cars. I think ABS is the number 1 item that can fail.

Mighty convenient that they had to keep unplugging the 12V. Even if a customer brought a reader they won't be able to find pending codes or other useful information, like long term fuel trim.

They kept the control but didn't remove the aftermarket remote start? Does the OP at least know what brand it is? Contact that vendor and ask them how to remove it (or just get the installation instructions and work backwards, assuming that some wires may need to be spliced back together). Otherwise, perhaps a trusted mechanic would be of some use at this juncture.

We have the remotes to the automatic start, we thought she had bumped it the first time it did it after we took possession.

Update: it continued to do it and we were able to get it to a Toyota dealer while the lights were on
they are saying all scans lean towards the transaxle, does anyone have experience with this?

Actual transaxle failures are very rare. Individual owners are unlikely to have experienced one. Damage outside the transaxle which makes it appear to be broken would be more common. Things like broken wires, corrosion at connectors, rodent damage, and so forth.

Reviewing this thread, there are no codes provided. The dealer told you what codes they read, yes? If not, scan it with a capable tool and report back what the codes are.

^+1 Please post the actual five digit trouble code(s) that the Toyota dealership found.

    P0AA6 is my vote......

There is no brake actuator failure that will prevent the car from going READY.
Allowing you to still drive the car when the codes trigger, but then locking you out of READY if the car is turned off is almost always a P0AA6, which is almost always the HV battery finding a path to chassis due to battery electrolyte leakage. Sometimes it's the transaxle failing, sometimes it's a damaged power cable, sometimes it's the AC compressor. Full codes and a little troubleshooting will help complete the story, but maybe that's why the dealer is saying TA.
