I had the main warning and check engine lights come on a week ago. The Prius continued to run normally. I took it to the dealer and they said they could find nothing wrong with it.

5 days later (today), it happened again and I took it back. This time they said they had learned that there is a Special Service Campaign (# 40 D) that requires reprogramming the computer. It took about two hours.

Here is what I found out about it on the web:
    _____________________________________
SSC 40D - 2004 MODEL YEAR PRIUS HYBRID VEHICLE ON-BOARD DIAGNOSTIC
    SPECIAL SERVICE CAMPAIGN

    What is the condition?

On certain 2004 Model Year Toyota Prius vehicles, the Hybrid Vehicle Electronic Control Unit (HV ECU) and Battery Electronic Control Unit (Battery ECU) may not conform to some portions of the diagnostic tool communication standard due to a programming error. If a repair facility's On-Board Diagnostic (OBD) scantool is used to diagnose a condition where the hybrid system warning light and/or master warning light is illuminated, the OBD scantool may not recognize the system fault.

    What will Toyota do?

Before you are inconvenienced by this condition, Toyota would like to reprogram the ECUs in your vehicle. Any Toyota dealer will conduct this service at NO COST to you.

    What should you do?

Please contact your authroized Toyota dealer to make an appointment to reprogram the ECUs as soon as possible. The labor time necessary to reprogram the ECUs is approximately one hour. However, depending upon the dealer's work schedule, it may be necessary to make your vehicle available for a longer period of time. The Toyota dealer will make every effort to assure your utmost convenience during this service.

In order to assure your full protection under the emission warranty provisions, it is recommended that you have your vehicle serviced as soon as possible. Failure to do so could be determined as lack of proper maintenance of your vehicle.

If you do a quick search under SSC 40D and variations on that you'll find lots of information and other threads. It is situations like yours that it was intended to address. Note, however, that I, and a couple others, have had wierd problems, since the SSC 40D, that still did not show a code....there's still a ghost in the machine....or more than one.

Previous to my post, I tried to see if there were other threads about 40D and only found one similar situation. Subsequent to my post, I found several other threads. I realize now that this is not a new topic, as I thought it may be.

Unfortunately it appears that it was new to my Toyota dealer (Darcars Toyota, Silver Spring Maryland). Had they known about it, they would have addressed it the first time I came in. Also, I had a 5K and 10K service at Jack Taylor Toyota in Alexandria, Virginia, and they made no mention of a need to comply with a Service Campaign. I think it would be useful if someone in these dealerships were interested enough to follow this web site to know what is going on with the Prius.

I'll be watching for gas guage and mileage issues, thanks to the postings of others.

    Thanks.

Evan...I have not had my 40D done yet; waiting for the 5k miles service. Can you explain what things you or others have experienced after the 40D was done? I am now warry of getting the 40D done but it looks like it is required by Toyota.

I don't think anything that's happened is BECAUSE of the 40D, but rather that there are yet unresolved issues with the code recording that is causing certain codes to not be recorded after an anomolous event.
