Can I get some opinions on what this problem may be?

It's your hv battery again. When replacing modules, #1 the modules need to be load tested to see how well they perform under load.

If the replacement modules were good, then the entire battery pack would need to be rebalanced when done. Also it's best to replace modules, 1 block at a time (1 pair of connected modules). That way the capacity levels on both modules match.

Thanks for the quick reply. Do you have a recommendation on what to do now? I'm not sure how to go about rebalancing. If I take the car to the dealer, do you think they'll try to sell me a new battery pack rather than fixing what's wrong with this one? I'm trying to avoid the dealer route if at all possible.

Dealer will just sell you new battery pack. If you’re keeping the car longer it’s probably best to buy new battery pack from Toyota. You can install it yourself to save some money.

you can get a grid charger from the priuschat shoppe, or by a rebuilt pack from a reputable company.
