EXTRA MILE tire warranty - anyone use this?

I just got a flat tire- conveniently after surpassing my regular 36k warranty. So no free roadside assistance. However the car came with the "Extra Mile" tire warranty package which I think is supposed to last 60k miles (with factory tire rotation every 5k).

Has anyone ever used this to get reimbursed for new tires? Does it cover fixing flats? I received hardly any literature describing the program, but it did include a card with a toll free number, which I will call first thing tomorrow.
