Almost all have it after the previous run had an incomplete warm up cycle. Was that your experience, too? The part you ref is an oxygen sensor.

You are the first that I have read about that had a Check Engine Light and DTC codes. How many miles do you have on your car? If less than 36k miles it should be covered under warranty.

The codes indicate the Heated O2 sensor that Skoorbmax indicates and then Random Misfire.

    P0138 - O2 Sensor (Bank 1 Sensor 2)
    P0607 - Control Module Performance
    P0300 - Random Misfire
    P0302, 0303, 0304 - Cylinder 2, 3 and 4 misfire

Interesting, could be a clue. The O2 sensor was the first (and only) engine component that failed in my belated Acura Integra.

After reading this post, I print out a few key post from here, took a Video of the event, took the car back to the same dealer, so far, they ask me to email them the Video so they can send it to Toyota. I had a schedule this coming Tuesday to turn my car in, they want to see if they can see the event the next mooring. They will have a field engineer from Toyota to come check out my car. Also change out the VVT-i and o2 sensor.

We have a 2002 with 112,000 miles - runs well but we have ALWAYS had the following: At start up the fuel pump cycles 1, 2 sometimes 3 times. If we try to start it before the 2 or 3 cycle it starts with SHAKE / KNOCKS - then runs fine. If it is not started for a week - then even if we let the fuel pump cycle 3 times - it starts roughly - even longer. We have been taking it to Toyota dealers in Calif & Washington State - no problem found they say. I had the gas tested 4 times - no problems there. This happens only on COLD starts and letting it sit for a week ? The O2 sensor checked out fine - the latest dealer says fuel pump is OK. damn it - it's been a great car otherwise . Any ideas ?

Bought our 2011 about a month and a half ago. We put 3200 miles on it (trip to Florida) and this morning went to start the car to go for breakfast. Had a very noticeable knocking from the engine for just about 3 seconds then went away. Wife told me it did this once before and it's always after the car sits for about 2 or 3 days.

So it obviously hasn't been resolved, which leads me to think Toyota still doesn't know what this is.

Nope, we brought it in and had it checked. They said they couldn't verify it. Just have it on record now if they do figure out a fix that we had complained about it.

It happened 2x to me. On the second occurence, once I put the car in gear, it stopped immediately. Now when I'm ready to go on a cold start-up, I never wait for the engine to start before I move the shift lever into gear and it hasn't occurred since. On initial start-up if cold, the car moves on battery giving the ICE a few seconds to warm up before engaging it for drive.

That's how I always did it. Power up, toss in gear. Another one of the nice things about Prius over a conventional.

Has this ever occured when the car is moving with cold start? Or warm start?

Like said above, pretty sure I always had that sucker in gear and moving before ICE kicked on. I'd usually have backed out of garage, in driveway, then ICE comes on.

Still, don't want to be rushed to put it in gear.

When I'm starting the Accord, waiting some seconds, I recall I'd be halfway down the street in the Prius.

It happens to my gen 3 on a very cold start (below 0 F ). By moving the shift lever to nuetral, the generator is disabled and does not load the cold engine. After the engine smooths out, I shift back to D then park so the generator reconnects and warms the traction battery a bit before driving.

Had it happen again and I KNEW it would happen, even had my father in law stand there so he could hear it.

This "event" is totally repeatable*, any dealership claiming they can't recreate it isn't paying attention or purposely avoiding it.

    *Make sure engine is cold, preferably in cool weather. Start and move car about 10 feet, ICE must come on at start-up and still be on when you turn car off. Let car sit over night. Start car next morning {{{insert bucket of bolts sound here}}}

This is EXACTLY the situation for my 3 events. 1st time was a surprise, 2nd time was an error on my part, 3rd time was almost on purpose to test my theory.

Well Sporin, your repeatable steps mirror what others have suggested, however, I don't think anyone has been able to recreate it at will. I have never had it happen once myself in 35,000+ miles. I did try essentially the steps you outlined to make it happen but it did not.

Take it to the dealer if you can repeat it and show it to a tech or service manager.

    Let us know the outcome.

If I ever find I can be without my car for 3 days then I just might. In the meantime, I'll be making sure I don't set my self up to repeat it at home.

This is how I tried to reproduce it and couldn't, though I may have waited only several hours. I do think that this is the best shot a person has at causing this event, though. It really does have to do with the engine shutting down while cold and not getting warm.
    Problem is he'd have to:

    1) Drive it there
    2) Come back next day when it's cold and turn it on
    3) Come back the next day!

And then even still they'll be like "Yep, I do hear it!" and that's that, because really it's not actionable. What can they do?

We're at 30k now and have had this happen twice, I don't think it's happened in many, many months since.

I agree that it might be an inconvenience but Sporin is the first person that I have seen on here that is able to reproduce it at will. Are we going to try to get it to a dealer and have it documented or are we going to just whine and bitch about it on PC?

Whine and bitch. What do you think the dealer is going to do? Some have "registered" this occurrence with their dealer, but nothing more.
