Multiple Codes, Starts fine, terrible auto shutoff.

    Here's whats thrown
    P3191
    P0171
    P0101
    (P0113 & P0102 - appeared after cleaning MAF sensor.)

    Noticed that the engine was reving high and not auto shutting off at lights or when parked, even well after the engine was warm. Initially the CEL codes were just P0171 and P0441. I checked the gas cap and found it was slightly loose, so I clicked it and cleared the codes. On the way home the engine seemed to function as normal, it auto turned off at when stopped at a light but about half way home the CEL came back on. I left it "on" while I found my OBDII scanner, after hooking it up it was just the P0171 code but then the engine started up and was surging and then when it tried to cycle off it knocked and sputtered pretty bad and then died. The check hybrid light came on.
    Rescanned and the above codes were present.

    Recent maintenance:
    Replaced spark plugs, new air filter and new cabin filter a week ago. Oil change, engine and inverter coolant change about 5k ago. Big thanks to @NutzAboutBolts for the tutorials.

    Steps I've tried:
    Cleaned MAF sensor with CRC MAF cleaner
    Cleaned throttle body with CRC throttle body cleaner

    My next step is to replace the MAF sensor.
    Then maybe investigate vacuum leak, perhaps at the Purge VSV, found TSB that indicated this was a culprit for P0171 and P3191
    I've seen some member have had similar issues and ended up replacing their fuel pumps...

    Thoughts Please?

    Some questions:
    • What spark plugs did you place into service?
    • How many miles since the spark plugs were swapped?
    • Have you tried pulling the 12 volt lead to reset the codes?
    • All connections are seated and snapped into place?

    1. I used NGK 4912s, they were in for almost 200 miles before the CEL came on, This is my wife's daily driver and she said she noticed that the ICE started idling high about 2 days (~170miles after spark plugs) before the CEL came on. No misfire codes though...
    2. I did pulling the negative lead off the starter battery, everything reset until the ICE started and died.
    3. All connections that I have touched are in their proper positions.

    The NGK 4912s are equivalent to the OEM denso plugs.

    I have used those plugs on several 2011s without any problems.

    Sensor came it today so I:
    Disconnected the negative lead on the starter battery.
    Made a phone call for about 30 minutes.
    Removed old sensor and installed new sensor.
    Reconnected starter battery.
    Started car.

    Same issue. ICE starts runs fine for 10 seconds or so then rough, sputters and dies.

    I really could use some advice on which route to go next.

    Went on to the next step from my OP and investigated the Purge VSV and low and behold the hose to/from? the intake has blown off.
    Based on other posts this vacuum valve seems to be a known issue but Toyota says the T-SB-0041-15 doesn't apply to my vin
    I put the hose back on and reset the codes.
    ICE runs without throwing codes immediately as it was before replacing the hose but there is the intermittent surging that was occuring before and the ICE isn't consistently shutting off at lights.

    The part that the hose came off of is 90910-12279 but it doesn't come up on Toyota's website.
    Looks like it should be 25860-37020? Can anyone confirm?

    Replaced the Purge Valve. Took it for an hour drive, mixed highway and city, and surging is gone, ICE auto stops at lights and when parked and no studder when ICE turns off.
    In other words there are none of the symptoms that were present before and the car functions perfectly.
    I'll update the thread if anything changes. I've got an 11 hour drive in two weeks so that will definitely be a test and I'll report back afterward.

    That's great to hear. Sometimes I wonder how post updating works on this forum or maybe my computer? My reply in Post 11 was directly for the #7 post, which was the last post my computer showed at the time. Yet this morning, it's showing an 8, 9 and 10. Very frequently, I'll see a post for the first time (under new posts) that is days old, even though it's the first time it's ever displayed. Anyone else run into that problem?

    I think in this case it's due to my being a new member. My posts (8,9) were all awaiting moderator approval when you posted. I certainly appreciate what good oversight can do for a forum though, some are a mess.

    i have also seen that happen. usually, you get the message, but not always.

    Drove 1200 miles this last weekend and everything ran perfectly.
    Had a standard interaction with the stealership. I just wanted to know if the software update for the previously mentioned TSB needed to be applied to my car, apparently I'm dense for believing that a TSB might need have the scope of VINs adjusted.
    They also said I needed an alignment, took it to a shop I trust and they said there was absolutely no need for any adjustments.

    If you've had any experience with this TSB and can verify whether or not I should pursue a software update I'd appreciate your insights.
