A torxse head screw driver pushed into the open slot and engaging with the teeth on the small gear wheel makes it easy to adjust up or down at will without having to bother with a ratchet. There are two slots that can be used one vertical and one at an angle visible in the pictures. This also works for the fog lights putting the driver through a hole below the fog lights in the plastic under tray.

2007 Prius and we get routinely flashed by oncoming drivers who think we have the brights on. Passenger headlight definitely higher than driver headlight and compared to my Ford F150, the lights hardly seem to illuminate the road at all, but I get a great view of the tops of trees.
So I used a box head ratchet wrench and it was super easy. do not know about torx head; I just got the concept just now.

So happy to be able to fix this. I can see the road now! Wish I knew about this fix before we drove across country in June!

John, thank you SO MUCH for this! My 2006's HIDs were pointing in different directions (vertically) and your post enabled me to finally fix them. I found the plastic adjustment nut but didn't know about the Torx trick. Bashed my knuckles for what felt like hours using a ratchet and socket on the little plastic nut and got no discernible difference. Then tried your suggestion and in less than 5 minutes had the headlights properly aimed. Brilliant!! You have saved my hands!! THANK YOU BRITPRIUS!!

    BTW I used a T40 Torx head.

Who knows if you'll ever get this, but I have a question about this. It seems like it doesn't actually work. I tried turning that nut many many times, and it doesn't seem like the headlights move at all. Is there anything else you can tell me?
