Keep in mind all the recommendations posted here are possibly Prius Folklore.
Every 50k miles you pop the hood and spend hours and hours (days?) cleaning this system?
That's approaching helicopter 'Maintenance hours / Flying hours'... (Not really,;) but this is a lot more than most cars.)
    And to what end?

Has there been reputable reports of Gen3 Prius owners that do all this 'Preventative Maintenance' and the head gasket is holding in there at +250k miles?

All these 'Preventative' Ideas are talked about a lot on this forum, but Toyota remains silent on the subject of Gen3 head gasket life span.

2012 gen 3 now with 345,000 miles, no burning of oil whatsoever.
I put this down to doing a engine flush every 3rd oil change, this flush cleans the piston rings and stops them sticking which in turn stops it burning oil which has a good effect on the EGR by not clogging up. Battery had done well till 340,000 then got the dreaded triangle of death, did a refurb myself replacing 4 cells and all is good now. Only other items replaced have been shocks, anti roll bar D bushes and brakes and tires.
Also 10,000 mile oil changes here using cheap fully synthetic oil.

Further evidence that, contrary to popular folklore, these cars don't all become oil burners and clog the EGR cooler as soon as they pass 100k miles. Mine hasn't either (yet!), albeit at only about 40% of your mileage.

Very impressive! What brand oil flush product do you use?

I’ve got a hunch @taxidriver50005 has intervened with his EGR once or twice.

I originally cleaned EGR system at 140,000 miles and haven't touched it since, I am tempted to pull it apart just for the hell of it but if it ain't broke!!!

I had never used the car scanner app before so hadn't a clue on what's good and bad on the flow side but here goes, 204520 miles since last EGR clean.

My wife also has gen 3 prius which we bought as an oil burner with 190,000 miles.
Treated it to a piston soak with same engine flush solution while we went on holiday then did engine flush as well, end result car does not need topping up between 10,000 mile oil changes

Check the pipe between EGR valve and intake manifold at the least; if it’s bad the rest is likely worse.

Amazing mileage ! Thanks for the info!
Did you buy the car new?
If not it might be that it has already had a head gasket issue and the replacement head gasket was the Felpro.

What was the piston soak product you used?
And this 'flush solution',, do you drive X miles with it in the oil, then change the oil?
What are the instructions on the flush solution?

That's interesting. What engine oil are you using, at what oil change intervals?

Have the EGR components ever been cleaned, or the head gasket ever replaced?

OK, 12.92 is getting down there. Squeaky-clean is usually up around 21 or 22. Mine was down to 10 point something before I cleaned it. Yours might be about due again.

Of course it can still go a lot lower before hitting the failing value and setting P0401. Whatever you're comfortable with.

Another possible cause of a low test score is a mashed ski jump in the EGR valve rotor. Also worth checking.

Wife car 245,000 miles, cleaned 50,000 miles ago..... To be fair I cleaned up EGR cooler really well but only scraped out the manifolds on both cars. Screenshot_20230126_170234.jpeg
