The head gasket has blown; the engine is next.

no code at all. I cleared them and they never came back since it no longer misfires. I do not need to smog untill June 2025

I like I said, no use replacing head gasket on an engine close to 300k miles when I can swap with JDM motor for $500 more. I figured this hack would give me a week or two, but four months later here we are.

I only ever cleaned the EGR and EGR cooler once and that was because I had EGR code and needed to clear it to smog back in June. In the past, I have cleaned just the throttle body, intake manifold and EGR pipe.

You may want to scan for pending codes before going to the smog shop. A pending code doesn't set the CEL until it happens a predetermined number of times, that's determined by the OEM.

I have a decent code reader and there are no pending codes for misfires. Car would pass smog. Strangely, I do see a replace hybrid battery code after swapping battery with one I bought of guy parting his car. I was getting Hybrid System Error messages and code with my last battery, but no Hybrid System Error with used one I put in. Not sure if the issue with installation or SW/ECU.

Check the floor for a leak , Sounds like a head gasket leak , into a cylinder ,does it run better after it warms up ?
My 2010 did this , I tried replacing the EGR but couldn’t get it off , and then started having this same symptoms , along with codes for cylinder misfires , then eventually the head gasket.
