Thank ypu. Changed the EGR valve and issue resolved. Took almost 2 years to get to this. But thank God back to driving like a dream....

I’m not sure it’d be physically possible, can’t imagine reaching both the fill and drain bolts. And without the engine underpanel...

Try cleaning the intake manifold? Note, in the attached, Toyota doesn't make any mention of of torque for throttle body hold-down...
