    Ideally?

Get the tires off the ground so they don't develop flat spots.
Do an oil change just before so no water in oil.
store the 12 volt battery on a charge maintainer for AGM using clips, 'cigarette lighter' plug is nonfunctional when car is off.
Have the HV battery mostly charged

    in practice?
if you skip all that you may need a new 12 battery. yours is getting up there anyway.

I would put it on a 12v trickle charger attached directly to the 12v battery and inflate the tires to the maximum allowed on the sidewalls. Ideally having someone start and drive it every two weeks would be nice. Some people might put it up on jack stands to avoid flat spotting the tires. The trickle charger should be a three or four stage unit, not that old one in the garage.

At minimum disconnect the 12 V battery. It should hold for three months with no load.

Park in Styrofoam blocks. Inflate tires. Oil change just before to reduce acids in gas. Park it after a longish high speed run of 20 miles. Disconnect battery (12v). Battery maintainer on battery. Put anti-rodent scents around and in car. Have a great trip.

Thanks everyone, I will put the battery on a maintainer charger, will have to leave it on the ground but will have fully aired up tires etc...
