OBDII shows all blocks at 16.4 or 16.5. 12 volt battery seems fine. HV fan running at speed 6, all the time. Where do I look next?

Once you clear the error code, for the most part the fan should have no reason to operate until the warning lights go on again...

Also there are subcodes to that primary code that can point you into more focus.

Which app are you using? Dr. Prius app is good for subcodes and real time monitoring.

It's relatively safe to clear the error code and keep driving as long as battery temperature doesn't heat up.

How do you know the battery temperature?

Thanks! I have an OBDII and I have installed Torque Pro on my phone.

I turned off all the codes, checked the all the blocks and their voltages are all in the good range.
I was checking it as I test drove. The HV Charge started at 30 kW then when to 20 kW and after a bit it jumped to 40 kW and then to 60 kW. At 60 kW the triangle and death lit up. Can anyone tell me what is going on?

Torque isn't specifically designed for hybrid cars and is problematic to many. It also doesn't read P0A80 subcodes that will allow you to do a diagnosis. Try Dr. Prius app instead and grab a screenshot of the battery monitoring screen, as well as let us know what your subcodes are and we'll help you out.
