I know Gen3 is a big issue, where the PCV thing can be solved by Oil catch can, EGR passage to intake should be cleaned oftern.
Does Gen4 also have the same issues?
My car seems to have cold start misfire. Gen4 swapped Gen3 car

Someone brought a Gen 4 to Tampa Hybrids, with around 300k miles on the odometer, and against their advice had them do an EGR cleaning. It turned out to be a waste of time; EGR interior had very little deposits. Toyota learned how to do it right, on the backs of Gen 3 owners.

Oil catch can is probably still a good idea; you could tell by pushing the throttle blade open, see what it looks like. A shiny pool of oil would be the tip off.

Toyota learned how to do catalyzed EGR, on the backs of years research that was conducted industry-wide and published during the gen 3 production years.
