Lots of good advice already, but I like to keep my driving rules simple:
EV = In-city and/or during stop and go on the highway
Eco/HV = Driving at highway speeds

I also don't bother with using B mode unless I'm going down a really (really!) long mountain/hill. The normal braking seems to do a good job of recharging, as long as you keep it at light braking rather than hard braking. From my experience with Hybrid Assistant, light braking regens the battery, hard braking uses the brake pads.

Similar here. Just keep the HSI indicator from hitting the end of its travel on the "Charge" indicator and there should not be friction braking.

Conservative helps, but where did that 15 mile EV range come from? 25-35 is typical. And it can make a huge difference when managed properly.
