Pianists: how can I play this measure?

    Top is treble clef, bass is lower. I have above average size hands but for the life of me cannot possibly reach all these notes, whether I play the lower three with left or not. Go try it, I don't think you can reach, either!

    There is a standard b-flat also in play.

    Also, as I haven't received any music training at all in about a century, if a given note receives a flat or sharp for a measure only (i.e. not at the start of the page), but later in the same measure it doesn't--does that flat or sharp still apply automatically, or must it explicitly be stated each time?

    EDIT: OK I can play this chord now :) Just had to put my hand up higher and I can manage to reach it, barely. Found some guy on youtube teaching the same song though and has a slightly different easier to play variant for this one.
