    Crud. i just realized that is the OLD form. So call the # on it and find out where you can download the latest application. Im suprised that they have not pulled that one off the site.

    As far as I know, I think that is still the current form. The only part that has changed is earlier in the document where it indicates the limit is $1000, which Ontario has since doubled to $2000. However, this amount is not indicated anywhere on the form itself.

    I filled that form out and mailed it around early/mid May (need to check my records), so I should be receiving my rebate any day now... Will report back when I do!
