Anyone knows if this part is correct for 2018 Prius?

They have a cheaper one but I'm not sure the difference

No. According to the listing in Figure 81-11, Rear Combination Lamp, both parts you linked (81561-47460 and 81456-47020) are used only on cars produced in December 2018 and later, for U.S. model year 2019, with the new, horizontal design for the rear lights, seen on page 1 of the 2019 brochure (PDF).

For model year 2018, look for “From 201805 to 201812” in the listing, and refer to the first of the two images. For the left side, the (upper) rear combination light assembly LH has part number 81561-47283, and the (lower) rear light assembly LH has part number 81591-47021.

It’s regrettable that parts.toyota.com says “This Fits Your 2018...” for parts that clearly don’t. That site is produced for Toyota Motor Sales, U.S.A., by a third party, SimplePart. They use the images from the Electronic Parts Catalog (EPC) produced by the Service Parts Engineering Administration Division at Toyota in Japan, but alas, the site doesn’t seem to use all of the applicability information from the EPC.

For 81561-47283 Upper part, does it only come with the housing and a screw?

The image shows the product breakdown structure, but it uses part name codes (PNCs), rather than showing most of the part numbers directly, so the image doesn’t have to be updated when part numbers change.

Part number 81561-47283 (PNC 81561D) includes the combination light assembly itself, a clip (part number 81559-47291, PNC 81559H), and according to note ※1, the “LED for Tail lamp.” I believe the socket and wire assembly, bulbs, cover, guides, screws, and other clips shown in the image are all sold separately.

An eBay listing for one offered by a Toyota dealer has pictures of the label and what comes in the box.
